The IN CFA5 Form is used by Committees that receive a contribution or makes an expenditure on behalf of a candidate. An expenditure is considered to be on behalf of a candidate if either of the following applies:
- The Expenditure is made in support of the candidate who is specifically identifiable.
OR
- The Expenditure is made in opposition to an opponent: of the candidate; and who is specifically identifiable.
This form is not filed with the Indiana Election Division or a county election board. This notice is to be given directly to the candidate’s committee.
